چکیده
Abstract Introduction Transcatheter Edge-to-Edge repair (TEER) for the therapy of mitral valve regurgitation (MR) is an alternative to surgery high risk patients. It a minimal invasive procedure but acute impact pre- and afterload decrease, due reduction both MR area (MVA), on left ventricle (LV) systolic function was never formally evaluated. Method Fifty patients with mixed etiologies from our TEER register complete 3D datasets LV before after were included in this study. volumes function, as evaluate by ejection fraction (LVEF), global longitudinal strain (LV-GLS) circumferential (LV-GCS), (3D vena contracta area, VCA) evaluated at end therapy. Results The majority suffered secondary (54%), one device implanted 64% two 36% population. leads significant all volume parameters excepted end-diastolic which increased non-significantly (Table 1). decrease LVEF linked GLS GCS multivariate analysis (r 0.85), having higher influence (beta −0.606 vs −0.435). In univariate linear analysis, LVEF, inverse proportional direct VCA. also directly MVA percental reduction. After evaluation, kept VCA (r=0.51, p=0.001), only (r=0.371, P=0.08) diminution (r=0.313, p=0.027). Conclusion TEER, ventricular seems principally increase (i.e. reduction); ventricles highest fractions being apparently maximally impacted. Funding Acknowledgement Type funding sources: None.

Acute Mitral Regurgitation Secondary to Papillary Muscle Tear: Is Transcatheter Edge-to-Edge Mitral Valve Repair a New Paradigm?
Severe mitral regurgitation (MR) secondary to papillary muscle tear is an infrequent but overwhelmingly morbid and often fatal complication of acute myocardial infarction occurring in 1% to 3% of patients. In-hospital mortality without surgical correction can approach 80%; however, even with surgical correction, papillary muscle tear carries a mortality rate between 19% and 53%. While considera...
متن کاملEdge-to-edge mitral valve repair without ring annuloplasty for acute ischemic mitral regurgitation.
BACKGROUND Alfieri edge-to-edge mitral repair has been used clinically with ring annuloplasty to correct ischemic mitral regurgitation (IMR), but its efficacy without concomitant ring annuloplasty has not been described in this setting. METHODS Seventeen sheep underwent implantation of 9 radiopaque markers on the left ventricle, 8 on the mitral annulus (MA), 1 on each papillary muscle (PM) ti...

متن کاملEdge-to-edge percutaneous repair of severe mitral regurgitation--state-of-the-art for Mitraclip® implantation.
MitraClip® therapy is a percutaneous edge-to-edge plication of the mitral leaflets, mimicking the Alfieri surgical technique. MitraClip® implantation is a safe procedure, and survival outcomes in high-surgical-risk patients are superior to historical controls. Despite these results, questions remain concerning long-term efficacy and durability. The MitraClip® device has been studied in a safety...
متن کاملEndovascular Edge-to-Edge Mitral Valve Repair
Background—The edge-to-edge technique is an accepted method for the surgical repair of a regurgitant mitral valve. This study reports the initial use of an endovascular technology that enables a double-orifice edge-to-edge mitral valve repair without cardiopulmonary bypass in an animal model. Methods and Results—Adult pigs (n 14) were anesthetized, and left thoracotomy was performed for epicard...
